Employee amputates finger on table saw

At 11:15 a.m. on September 16, 2004, Employee #1 was working with a Powermatic table saw in the production area, cutting very thick cardboard used to wrap around the glass products produced. The facility involved in the accident is a glass and glass coating manufacturer. Employee #1 made contact with the guarded blade of the table saw, resulting in an amputated left index finger.
