Investigation Summary
At approximately 7:30 a.m. on September 7, 2001, an employee working for Logoluso Farms was given the task of servicing the solution mixer located at Pump number 5. The employee turned on the mixer which did not start. With the power still on, the employee took off the guard cover that exposed the chain and pulley system. He started to grease the sprocket and chain. The motor started and the chain assembly was activated amputating the very tip of the employee's finger off. He was treated for his injuries at the hospital and then released.
