Investigation Summary
At approximately 4:15 a.m. on June 6, 2002, Employee #1 was wearing rubber gloves while using a scrubber pad to clean a waste sorting conveyor belt. He finished with the top of the conveyor belt and was cleaning the bottom of the belt when his left rubber glove became caught between the belt and the pulley intake. There was no guard to prevent contact with the pinch point between the belt and the pulley. Employee #1's arm was pulled into the machine and he sustained multiple fractures and was hospitalized.
