Investigation Summary
At approximately 2:30 p.m. on May 6, 2002, Employee #1 and a coworker were using a Komatsa forklift (Model #FG25T12), rented from United Rentals, to move some furniture. Employee #1 was riding on the fork lift next to the coworker, who was operating it. On about their third trip, Employee #1 got off the fork lift and took approximately three steps when the rear end of the fork lift swung over, and the back right wheel ran over Employee #1's foot. He sustained a fractured right ankle and was transported to the hospital, where he was treated and released after about 21 hours.
