Investigation Summary
At approximately 8:00 p.m. on March 25, 2002, Employee #1 was hanging a sign. The top of the flexible sign was 10 ft high and hung on nails that were in a beam. He was using a 6-ft wooden stepladder and had hung one end of the 10-ft-wide banner. He repositioned the ladder to the center of the banner, by placing the ladder between a table and a planter tub. He was on the fourth step and was starting to move to the fifth step when the ladder fell. He fell on the table and received head injuries.
