Employee Killed When Struck By Falling Bundle Of Pipe

At approximately 10:20 a.m. on December 18, 2001, Employee #1, a truck driver, was delivering a partial load of plastic pipes to the site. The trailer flatbed was 42 ft long and had a rear load of large 20-ft long plastic pipe. The front part of the load was various sizes of 20-ft long plastic pipes with the top portion going to this site and the bottom portion going to another site, except for one bundle of small 20-ft long plastic pipe that was on the top of the load and going to the second site. The rear part of the load had been loosened and was being unloaded by a forklift. Employee #1 loosened the front part on the driver's side and then went to the passenger side to complete the job when the bundle of small plastic pipes weighing approximately 1000 pounds fell and hit Employee #1 in the head. Employee #1 was killed.
