Employee Amputates Finger In Heat Sealer

At approximately 3:30 p.m. on March 28, 2003, Employee #1 was servicing the heat sealer indexer on assembly line 3 by tightening a screw on the base of heat sealer using a wrench while the machine was running. The heat sealer was manufactured by FEMC Food Equipment, and its rating was 480 volts, and 60 to 75 rpm. He was holding the screw with his left hand, while he tightened it with his right hand. After tightening the screw, Employee #1 withdrew his hands from the base of the heat sealer, and the sprocket chain drive of heat sealer, which was located near the screw, severed his left index finger. The diameter of the sprocket was 7 inches, and the center of the sprocket drive was located 9.5 inches from the cover plate and 30.5 inches from the floor. Vernon City Fire Department arrived at the scene with paramedics, and Employee #1 was transported to the Los Angeles County USC Medical Center, where he was treated and released. The accident was caused because Employee #1 did not de-energize the machine prior to servicing the heat sealer, nor were extension tools provided and used should the heat sealer need to be running while being serviced.
