Investigation Summary
At approximately 7:10 a.m. on October 9, 2002, Employee #1, a production supervisor for Sara Lee Fresh, Inc., was standing on the metal frame of the English muffin line and was cleaning the hopper in the make-up department while the machine was running. The metal frame was 32.5 inches from the platform and the hopper opening was located 41 inches from the frame and it was 10.5 inches by 16.87 inches. The hopper was manufactured by Bakery Systems, Model Number K400S and Serial Number 99225. The employee was coming down from the metal frame to the platform to change the flavor when his feet slipped. He tried to regain his balance by holding onto the dough pump with his right hand and grabbing the hopper with left hand when the rotating blade in the hopper severed the tip of his left middle finger just below the nail bed. He came down to the platform and noticed his fingertip was amputated. He turned off the machine and called the sanitation department to clean the machine and to look for his fingertip. Employee #1 was taken to the Stacy Medical Center where he was treated and released the same day.
