Investigation Summary
An employee was installing a window frame on the second-story floor of an elementary school that was being renovated. He was standing on an 18-inch ledge that was located between the first and second floors on the outside of the building. The owner/supervisor would visit the site occasionally, but was not on-site at the time of the accident. The leadman had some direction and control, but did not have management authority. This school had over 25 windows to be replaced and these same procedures had been performed numerous times. With due diligence, the supervisor could have know about the hazards. The employee stated that he must have misstepped and fell between 9 and 10 feet to the ground. He was hospitalized with a broken leg. The use of fall protection was not stressed or given to employees.
