Investigation Summary
At approximately 3:00 p.m. on September 19, 1998, an employee was injured while operating a forklift at a packaging and distribution facility. The employee had been working as a packer and stacker for approximately three months, and had no similar experience previously. The employee was driving the forklift to the outside of his building to pick up pallets. As he turned the corner, the truck overturned and the employee's left hand was crushed between the top rail of the forklift cab and the pavement. At the time of the accident, the employee was not an authorized forklift driver and had been previously disciplined for such action. The employee had been assigned to sort packages on the day of the accident, which did not involve operating a forklift. Operating forklifts is prohibited by untrained employees, and the employee knew the policy. The employee was attempting to facilitate stacking sorted material. He stated that he was being instructed by a lead man; however, no lead man was confirmed by management's accident investigation.
