Investigation Summary
On March 1, 2001, an employee was working as a laborer and handyman at the sound stage construction site. He arrived at the site in preparation for installing the insulation and finishing up of the remainder plywood floor decking. The supervisor and employee used a telescoping boom lift to access the floor decking located at 44 feet off the ground. The employee was told to do general cleaning of the roof deck including putting spacers on top of the wooden joists. At 10:30 a.m., he had completed putting six spacers on east section and five on the west section. The employee was working on the last spacer before placing the plywood. The supervisor and the other employee heard a "thug" sound and looked back but did not see anything. The supervisor called for help. Another employee from the office arrived at the site to attempt CPR on the employee until the emergency medical services arrived. The employee died of massive head injuries that were sustained in the fall.
