Investigation Summary
At about 9:15 a.m. on January 3, 1999, an employee of Home Depot wasresetting a stock shelf and adding an additional shelf 9 ft from the concrete floor. The shelves were large enough for the employee to walk on while adding the additional shelf. He fell from either the shelf or the ladder, striking his head on the concrete floor, and was killed. The employee was not wearing fall protection, and inspection of the ladder revealed the ladder frame was bent.
