EMPLOYEE DIES WHEN NECK IS CRUSHED IN SCISSOR LIFT

On February 25, 1998, a supervisor of Employee #1 instructed him to remove plugs from a conduit sleeves on the first floor near the entrance of the building. The supervisor left Employee #1 to check on other jobsites. At approximately 8:30 a.m. a supervisor of Coan Construction Company, another contractor at the jobsite, came by and saw Employee #1 in a scissor lift which was rented by Coan Construction. Employee #1's neck was crushed between the railings of the extended scissor lift and the horizontal portion of a concrete opening. Paramedics responded and brought him to LA County USC Medical Center where he died the next day of brain asphyxia. Employee #1's supervisor stated that he had a wooden ladder to access the conduit sleeves. It was also noted that it was a common practice for contractors to borrow other employers' equipment when it was needed with no prior requests or permission.
