Employee's hand injured in packaging machine

At approximately 4:50 p.m. on March 15, 2000, Employee #1, of Choice One Foods, was working on the vacuum pack machine when she noticed a piece of a sheet that had come off line. She removed a cover plate and reached in through a 1 7/8 in. by 16 1/2 in. opening to straighten it. Before she could complete the adjustment, the ram came up and caught her left hand between the seal die and the heating plate. Either a coworker or Employee #1 hit the EMERGENCY STOP button, freezing the ram in position so that her hand remained clamped under it. A mechanic from the maintenance department had to disassemble the machine to release Employee #1's hand. She was transported by ambulance to the USC County Hospital, where her hand was treated for serious lacerations, fractured bones, and burns on her fingertips. Had the machine completed its cycle, her hand would have been released within 10 seconds of being trapped. Lockout/tagout procedures were neither followed nor clearly posted on or near the machine. There was no procedure established for deenergizing the machine before the plastic rolls were to be changed. The opening was unguarded and there was no barrier near the heating plates and the sealing die. The machine had no limit switch in contact with the cover plate, so when the cover plate was removed, the machine was still operational. The employer was cited for a serious violation of T8CCR 3314(a).
