Investigation Summary
At approximately 8:15 a.m. on May 7, 1997, Employee #1 was guiding a load of lumber onto his truck when he was accidentally struck by a 3000 lb Hyster 180 forklift. He suffered a fractured right leg and multiple injuries to his leg and left shoulder. The employer removed the forklift from the premises following the accident.
