Employee killed when head caught in forklift mast

Employee #1 was riding on the forks of a lift truck when he stuck his head between the mast supports and the frame of the forks. The operator then actuated the lift mechanism. As the forks went up, the frame of the fork unit and the upper crossmember of the mast formed a catch point, crushing Employee #1's head. He was killed.
