Investigation Summary

On October 15, 1997, Employee #1, an Elite temporary worker assigned to Adco, Inc., was shown a video on forklift safety made by his assigned employer. After passing a written test, he began operating a forklift. Employee #1 was reversing the forklift to put stacked materials on the forks when he lost his balance and fell. His left arm became caught between the forklift frame and a wall, tearing a tendon and artery. He was inexperienced in forklift operations and had not been properly trained by the temporary employer. At the time of the accident, Employee #1 had been using his left hand to control the material stacked on the forks.
