Investigation Summary
At approximately 3:37 p.m. on March 18, 1998, Employee #1 was pulling record boxes to fill orders. Employee #1 was located in an aisle of the facility at a height of approximately 11.5 ft while pulling a records box from the fifth level of a multi-level shelf. After pulling the box from the shelf, Employee #1 stepped back in order to process the order. The pallet which is normally on the forks was not there and Employee #1 fell 11.5 ft to the concrete warehouse floor. Employee #1 sustained a broken pelvis, broken right hip and broken right ankle. Employee #1 was not wearing a safety harness.
