Investigation Summary

At approximately 10:14 a.m. on August 14, 2006, Employee #1, an owner of the company, was trimming some branches which had grown over the property's wood fence onto the sidewalk and road. At the end of the day, Employee #1 walked to the chipper on the sidewalk to turn it off. A small branch (2 in. in diameter, 10 ft long) that was approximately 12 ft to 15 ft overhead, broke and fell on Employee #1's head. The Employee #1 suffered a serious head injury and was pronounced dead at the hospital after few hours.
