Investigation Summary
At approximately 8:30 a.m. on October 20, 2006, Employee #1 worked for a company providing onsite deputy inspectors of construction activities for codes and quality compliance. Employee #1 was on the eighth floor performing the final inspection of the steel before spraying the fire proof coating. The fire proofing hose or the coupling detached from its connection and it whipped Employee #1's left foot. He was hospitalized with a fractured left ankle.
