Investigation Summary
On October 3, 2002, employee #1, the foreman of a four-member landscaping crew, was working to cut down approximately 200 unhealthy Eucalyptus trees from two, City of Los Angeles, 18-hole golf courses. These trees varied in height between 50 to 70 feet with diameters ranging from 20 to 24 inches. The city had marked the trees that were needed to be cut. At approximately 7:45 a.m., a 50-foot-high Eucalyptus tree being felled by a coworker hit employee #1 and killed him. An investigation of the accident revealed that prior to felling tress; workers were not cleared beyond the hazardous area (tree's striking range) to a safe zone beyond which they could not be hit by falling trees. In addition, it was discovered that employees did not utilize audible warnings or prearranged two-way hand signals before commencing "back cutting."
