Employee's Fingers Are Amputated in Belt and Pulley

On August 19, 2006, Employee #1 was changing a broken belt on Tubular Drive Belt Motor (TDBM) Number 3. There are four TDBMs located in very close proximity to each other on a mezzanine that measured approximately 14.5 ft by 7.5 ft. As Employee #1 turned to pick up a belt, he slipped on the broken belt. He caught the mezzanine perimeter guardrail with his right hand and his left hand came into contact with the moving, unguarded belt of TDBM Number 2, which was approximately 26 in. from the guardrail. The belt pulled Employee #1's hand into the nip point between the moving belt and pulley, the arbor of which was approximately 15 in. above the working level. He was hospitalized at Arrowhead Regional Medical Center in Colton for an amputated left fifth finger.
