Investigation Summary
At approximately 8:15 a.m. on March 22, 2010, Employee #1 was cross cutting a piece of luan (plywood) approximately 21 inches by 60 inches through a Rockwell table saw when it bound and spun around and hit his right index and middle fingers resulting in a fracture and dislocation respectively. Employee #1 was transported to Northridge Hospital where he was treated and released on March 23. Employee #1 was building sets and cabinets as a prop maker at a movie and television production construction mill site. According to Employee #1, the saw guard and the antikick back devices were installed on the saw. Employee #1 was employed by Couples in Studio City, CA.
