Investigation Summary

Investigation Nr: 201164613
Event: 02/22/2003
Employee Killed When Falls from Garage

On February 22, 2003, Employee #1 was part of a construction crew working on the construction of a 13-story new "Cal Trans-District 7 Headquarters" building. Employee #1 fell approximately 28 feet from the P1 level to the P3 level. He was last seen working between grid line Numbers 7 and 8 and J and X on the P1 level. His unconscious body was found on the P3 level directly below the 24-inch by 123-inch floor hole on the P1 level. Coworkers contacted the Los Angeles City Fire Department. Paramedics responded immediately and found Employee #1 on the ground, non responsive, with major head trauma and multi-system trauma. The paramedics transported him to USC/LA County Hospital, where he died shortly after arriving. The Medical Examiner's Report stated the cause of this accidental death was due to multiple blunt force injuries, both to his head (extensive fractures, brain contusions, and hemorrhage) and left chest (multiple rib fractures, hemothorax, and pneumothorax).
