Investigation Summary
At approximately 10:30 a.m. on March 8, 2005, Employee #1 was painting the roof of a motel complex from an unsecured 8-ft aluminum ladder. He had positioned himself on the fifth rung of the ladder when the ladder slipped and fell. Employee #1 fell off the ladder and from an elevated structure, falling approximately 15-ft to a concrete surface. The impact of his fall caused loss of consciousness and difficulty in breathing. Emergency medical personnel transported him to a local medical center, where he received medical attention for severe head trauma and scalp laceration. The employee was in a coma for five days. Once he awoke, he was hospitalized for a month for additional medical treatment and care. The causal factor that lead to the accident was not securing the ladder while in use, which is contrary to the manufacturer's recommendation.
