Investigation Summary
Employee #1, working for Mercury Electric in Hollywood, CA, and a coworker were assigned to install a lighting fixture for Otto Inc. The lighting fixture they needed to work on was about 28 ft high. In order to reach this height, they set up three sections of rolling-type scaffold without rollers on it and put an extension ladder on the top plank. The height of the scaffold was 15 ft and the ladder on the top plank gave an additional 10 ft of reach. Employee #1 climbed the ladder to work on the lighting fixture while his coworker remained on the ground to assist him. As Employee #1 was installing the fixture, the scaffold tipped back and the ladder fell. As a result, Employee #1 fell and landed on his feet and back, sustaining broken bones on both ankles. He was hospitalized due to his fractures.
