Investigation Summary
At approximately 10: 00 a.m. on February 11, 2006, Employee #1 was working from a 16-ft extension ladder, trimming an oak tree's lowest branch, which measured 12-ft by 15-ft by 4-inches. The branch came down before Employee #1 had finished the cut, due to the heavy weight at the tip of the branch. When the branch came down, it hit the bottom of the ladder and caused him to fall six feet. His coworkers called for help and he was taken to the UCLA Westwood Hospital, where he was hospitalized for several months, as a result of his injuries.
