Investigation Summary
At approximately 9:30 a.m. on October 11, 2006, Employee #1, a hospitality coordinator, was walking toward the visitor center from the adjacent parking lot. She was carrying a box of several items in her arm, when she arrived at the front stairway of the visitor center. Employee #1 placed the box on top of the brick flowerbed and stepped forward. As she descended the stairs, she tripped on the bottom stair. Employee # 1 lost her balance and hit her forehead on the handrail of stairway. Then, she fell backward, hitting her head on the concrete ground. She sat up by herself holding onto the handrail. A worker form another establishment heard the crash and came out to find Employee #1 on the ground. He went into the visitor center to inform a coworker about what had happened. Emergency medical personnel were summoned. The coworker came outside and stayed with Employee #1 until the paramedics arrived at the site. The paramedics transported Employee #1 to a medical center, where she remained hospitalized until she died ten days later.
