Investigation Summary
At approximately 11:50 a.m. on August 11, 2005, Employee #1, a carpenter of the Jovel Wood Creations, Incorporated, was engaged in manufacturing of wooden home furniture. The operations included cutting, assembling, sanding, spray painting, and packing. Employee #1 was cutting and dadoing frames of a wood cabinet on a table saw, which was equipped with three blades. In addition, a guard was pinned back for the dadoing operation. When Employee #1 placed a work piece over the saw blades for dadoing, the work piece kicked back and his left hand was cut by the saw blades. Employee #1's fingers were amputated. Employee #1 wrapped his hand with his shirt and walked over to his supervisor, who was working at his workstation about 25 ft away. The supervisor took Employee #1 to Western Hand Center, where he was diagnosed with multiple traumas to his left hand and with bursting lacerations. Employee #1 was treated and released on the same day. The accident happened because combs or suitable jigs were not provided and a standard guard could not be used on the table saw for dadoing operation.
