Investigation Summary

Investigation Nr: 202451712
Event: 08/15/2005
Employee Amputates Finger While Cutting Meat

At approximately 3:30 p.m. on August 15, 2005, Employee #1 was working as a apprentice meat cutter at an Albertsons grocery store. She had worked for Albertsons for five years. She was in the meat department, using a WestGlen-Cobra 16 band saw, with Serial Number KA 160290, to cut the bone-in country-style pork strips. She had not adjusted the guard on the saw blade as close to the table as possible. Employee #1 was holding the pork with her right hand as she cut it on the band saw. Employee #1 was putting the pork strips on the tray, when she noticed that her right ring finger had been cut by the saw blade. Employee #1 called a coworker, who was taking care of the butcher block, for help. The general manager came over and took Employee #1 to Glendale Memorial Occupational Center, where she was treated and released on the same day. She was not hospitalized. Employee #1 had sustained a complex laceration of her right ring finger with loss of the nail bed, nail and tip of the distal phalanx. A causal factor was that Employee #1 was operating the band saw without adjusting the guard on the saw blade as close to the table as possible.
