Investigation Summary
On April 8, 2005, Employee #1, a facility service man, had finished washing a truck in the transportation department parking area. He gave his coworker the required "all- clear" signal to indicate that he was clear of the vehicle and that it was safe to move the truck from the washing location back to its designated parking spot. The employee was standing in front of the truck so that he could be seen by the coworker. When the coworker started to move the truck forward, Employee #1 noticed that the hose was becoming entangled by the wheel. He attempted to get his coworker's attention to stop; however, before he could, he became caught in a loop of the hose and was thrown to the ground. He landed on his right arm in an awkward position, which resulted in a fracture. Employee #1 was transported to Providence St. Joseph Medical Center in Burbank, California, where surgery was performed. He was hospitalized.
