Employee injured in fall from fork lift forks

Employee #1 fell from the fork lift forks, falling 9 feet to the concrete floor. Employee #1 was removing product from shelving. Employee #1 was standing on the forks of a fork lift when he lost his footing and fell to the cement. No safe work platform was in use during the job assignment. Employee #1 was hospitalized with a fracture.
