Investigation Summary
At approximately 11:00 a.m. on July 16, 2000, Employee #1 was in the stockroom at a Sears Roebuck and Company facility, preparing materials for an inventory count. She ascended an 8 ft ladder stand and then onto a 32 in. wide by 10 ft long by 1/2 in. thick piece of particleboard on heavy metal shelf frame with a 3/4 in. lip. Employee #1 was on the board when it bent and slipped off the metal lip. She fell 10 1/2 ft onto the concrete floor, fracturing her upper left leg. A coworker supervisor was in the stockroom at the time of the accident, performing the same type of work.
