Investigation Summary
At approximately 11:15 a.m. on June 21, 2000, Employee #1, of Jeffery's Auto Body, was preparing a vehicle for painting. He needed a ladder to reach some of the areas on the vehicle, so he retrieved a nearby 6 ft aluminum stepladder that was nearby and set it up by the vehicle. He had been working from the second or third step for between 5 and 10 minutes when the ladder gave way and he fell. Employee #1 suffered multiple contusions to his head and back, for which he was hospitalized.
