Employee injured when finger tip amputated by forklift

At 9:00 a.m. on May 13, 1999, Employee #1, a volunteer for this employer, had the tip of his finger amputated. He had driven a truck from Lompoc to Santa Maria to pick up a load of food. As the truck was being loaded with a forklift, Employee #1 had his hand on a grab bar. The pallet of the forklift came too close to the grab bar and a piece of wood punctured his finger. Employee #1 was hospitalized for treatment.
