Investigation Summary
At approximately 11:00 a.m. on April 28, 1999, Employee #1 slipped and fell breaking her lower right arm. Employee #1 was cleaning room 302, on an apple farm, behind the toilet bowl. As she had completed this task, she was moving out from behind the toilet bowl and next to the tub. She slipped and fell with her right arm hitting the tub. The floor was a tile floor and it was dry. Employee #1 was wearing flat shoes with rubber soles. She was taken to the hospital where she stayed 23.5 hours.
