Investigation Summary
At approximately 3:40 p.m. on January 21, 1999, Employee #1 was operating a tractor (called a mayote) to move a trailer when he got caught in between them. The tractor is a motorized transfer trailer that is operated by a pendent as an employee walks along the side of it. Employee #1 was standing on a service ladder of the tractor as he operated it to pull the trailer. As he made a turn, he backed up the tractor and collided into the tongue of the trailer crushing his left leg. He sustained fractures to this leg and was hospitalized due to the accident.
