Investigation Summary
At approximately 11:00 a.m. on December 24, 1998, Employee #1, a supervisor on the job site, was making corrections on a stairway. He selected the 2-ft-2-in. by 12-in. planks and placed them across an 8-ft span. Employee #1 was on the planks for about two minutes when one of the planks broke and Employee #1 fell 10 ft onto a rocky surface. He sustained fracture of the lower back and was taken to the hospital. The planks used were not the scaffold planks. The plank that broke had a wood grain that ran across the plank and that caused a weak point where the plank broke.
