Investigation Summary
At approximately 2:00 p.m. on October 28, 1998, an employee was working on a roof at a chicken barn. His job was to remove the copula, a raised portion of the roof at the peak for ventilation. As he was doing this, he slipped on the tin roof, which was slopped 3 horizontal to 1 vertical. The employee fell on his back and remained on the roof. He injured his back. The employee was hospitalized and treated for sprained back.
