Investigation Summary
An employee was operating a Caterpillar TE83 reach forklift. The front wheels became lodged in a hole and he attempted to back out of the hole. The forklift stalled and slid down the dry lake bed excavation. He was wearing his seatbelt and suffered only minor injuries.
