Investigation Summary
Employee #1's hands were caught in the conveyor of a "Douglas" wrapping machine while he was installing a new roll of plastic. Employee #1 did not use the company's policy of lockout, tag-out prior to changing-out the rolls. Employee #1 sustained the amputation of the tip of his left finger.
