Employee injured when struck by falling tree limb

On May 24, 1999, Employee #1 standing under a large ash tree being trimmed and was struck in the head by a falling tree limb. The tree limb measured 3 ft long by 3 in. in diameter. Employee #1 was taken to the hospital and hospitalized. No head protection had been worn.
