Investigation Summary
At approximately 8:30 a.m. on August 30, 2017, Employee #1, a Pre-Load Supervisor, attempted to retrieve a package from inside the PS-1 pinch point roller conveyor system at the in-running nip point. The employee opened a section of a guard while the conveyor belt roller system was in operation. As he reached inside to retrieve the package, his right arm became caught in the conveyor and was amputated from the shoulder.
