Platform Lift Overturns - Employee Suffers Several Fractures

At approximately 7:00 a.m. on January 8, 1999, Employee #1, an electrician of KCE Electric, Inc., experienced a broken left shoulder, cracked pelvis, and a sprained thumb after a platform lift overturned. At 10:00 a.m. on January 8, 1999, the Los Angeles Sheriff Dept notified the division and his employer that a serious accident had occurred. The incident occurred at a warehouse that was under construction in Paramount, California. Employee #1 was operating a Wildcat battery operated scissors lifting platform. The Model Number was 21-30, and the Series Number was 1200. Employee #1 was installing an EMT conduit in the overhead of the warehouse. Employee #1 just began working and had positioned the lift platform at about 15 feet above the floor. He pushed the joy stick forward and began moving the platform lift in an easterly direction. At that moment, he felt the left front wheel go into one of the holes. The lift teetered and then toppled over hitting the north wall. Another employee was about 150 feet away. He heard the crash and ran to the scene. He did not see the accident. Employee #1's supervisor also did not witness the incident. Employee #1 was transported by ambulance to Saint Francis Hospital in Lynwood, California where he remained hospitalized for 7 days. He suffered a broken left shoulder, cracked pelvis, and a sprained right thumb. A level was used to determine that the floor was in fact level. There were, however, 12 holes in the floor. These holes were used for sewer connections. Each hole was marked with an orange pylon. The holes formed a straight line from west to east and measured about 22 inches in diameter and 7 inches deep. The surface contained irregularities, and investigators believe these irregularities may have caused the lift to overturn. The operating manual and a placard attached to the platform provide warnings of these irregularities as well as other warnings.
