Investigation Summary
At approximately 11:00 a.m. on May 15, 1998, Employee #1, a forklift driver for Modine Western, was off loading merchandise from a freight truck parked adjacent to the loading dock. He was backing an unloaded forklift toward the loading dock when the vehicle smashed through the dock railings and fell into the 4 ft deep dock pit. The forklift overturned as it landed and battery acid splashed into Employee #1's eyes, causing partial loss of vision. He later stated that he tried to stop the forklift, but the brakes failed. There were no witnesses to the accident.
