Employee's finger fractured in chain and sprocket drive

At approximately 12:10 p.m. on October 14, 1998, Employee #1, of Santee Dairies, Inc., was in the corrugated packaging area, adjusting the drive wheel of a turntable machine when his right hand became caught in the pinch point of the chain and sprocket. The power supply was not disconnected and the machine was running at the time of the accident. The employer's safety supervisor stated that Employee #1 had not followed the company's safety rules on lockout/tagout procedures prior to adjusting the drive wheel of the turntable. Employee #1 stated that the power was to be kept on during adjustment work so that checks could be made during the procedure.
