Employee Left Hand Caught in Dough Conveyor Machine

At 8:15 p.m. on September 1, 1998, Employee #1 was operating a PAN O MAT 410, dough conveyor machine. Employee #1 attempted to remove a piece of dried dough ball while the machine was running. Her left hand became caught in the running rollers which severely lacerated her index and middle finger of her left hand. Employee #1 was hospitalized for severe lacerations. Employee #1 failed to stop the machine or use the extended tool to pick up the dough ball. Employee #1 stated that she did not know the location of the stop button.
