Investigation Summary

Investigation Nr: 170903942
Event: 08/05/1998
Employee's leg lacerated in fall from molding machine

At approximately 1:30 p.m. on August 5, 1998, Employee #1, of CCL Plastic Packaging, was on top of molding machine #16, changing the color on a console panel unit. He apparently lost his balance and fell approximately 39 in., striking the metal edge of a conveyor on the floor and sustaining a laceration to his left thigh. According to the employer, Employee #1 had not followed the usual route for going up the machine, which is through a stairway at the back of the unit. Instead, he took a shortcut by climbing up the front, and was standing on a 5 in. wide deck at the top front, from which he fell. There was a wide space at the back where Employee #1 was supposed to stand and work. There were no witnesses to the accident.
