Investigation Summary
Employee #1 was using a blanking saw to cut oak lumber. He was removing scraps that had become lodged under the blade guard with his left hand. The blade was activated, amputating his fourth finger and partially amputating his third finger. Employee #1 did not recall how the blade was activated. However, the saw had a foot operated switch that did not have a guard around it at the time of the accident.
