Employee injured when fell from a ladder

At about 11:30 a.m. on April 27, 1999, a serious injury occurred in Glendale, CA. Employee #1 was installing drapery for a theatre stage. His employer was engaged in manufacturing and installation of drapery for theatre stage. At the time of accident Employee #1 was climbing down a 20-ft-long extension ladder, which was leaning against a concrete column. After adjusting a drape on a steel truss, as he was descending the ladder, the ladder slipped backward, and Employee #1 lost his balance and fell about 8 ft to the concrete floor, landing on his chest. He sustained fracture of his left elbow, a cut on his chin and on left thumb, and a minor fracture on his right elbow. Employee #1 was taken to Saint Joseph Occupational Health Center where he was treated and released on the same day. Surgery was performed on his left elbow two days later.
